Stuck with a Shark Fin? Complete Tribal Medicine in Path of Exile 2 Act 4 and Get a Level 11 Gem

If you picked up a massive Shark Fin while exploring the Karui Archipelago in Path of Exile 2 Act 4 and then wondered what to do with it — you’re not alone. The fin is a quest item that can’t be discarded, so you’ll want to hand it in as soon as practical to free up inventory space and finish the “Tribal Medicine” quest.

How to use the Shark Fin in Path of Exile 2

The Shark Fin drops from the Great White One boss on Whakapanu Island. Once it lands in your inventory, the “Tribal Medicine” quest is added automatically and the fin cannot be removed until you hand it in.

To complete the quest, teleport back to Kingsmarch and speak to Makorū on the boat. Choose the option to Set Sail and pick Ngakanu as your destination. When you land, walk into the village and go right to find Kaimana, who accepts the fin.

If Kaimana isn’t on Ngakanu yet

In some playthroughs, Kaimana won’t accept the fin on Ngakanu until you finish the quest titled “The Search”. Consequently, you may need to complete more of Act 4 before you can hand the item in.

After finishing Act 4, Kaimana can be found back in Kingsmarch — specifically on a boat southwest of the one you use to sail around the Karui Archipelago. Deliver the Shark Fin to her there if you couldn’t on Ngakanu.
